Scarpa’s fascia provides an alternative blood supply that flows through from the groin vessels up into the upper abdominal flap and provides extra blood supply. In addition, instead of undermining the upper abdominal flap, Dr. Saldanha actually liposuctions this entire area extensively, which not only reduces the thickness of the flap, but also allows it to slide to the lower abdomen in the exact same manner as the undermining technique did in the traditional operation.
